COVID-19 Vaccination Requirement: All patients must be fully vaccinated against Covid-19. If you have an appointment schedule before you have completed your vaccination routine, I kindly ask that you reschedule you appointment to a time after completing your Covid-19 vaccinations. Individuals are considered fully vaccinated two weeks after their final inoculation. Please bring a copy (picture of card on phone is acceptable) of your inoculation card to present at your next appointment. I will have my inoculation card on hand as well.
*Masks are still mandatory at all times.*
COVID-19 Protocols: Please be aware there will be additional measures implemented to decrease the spread of COVID-19 including wearing face masks at all times during your appointment. Please bring your favorite KN95 or N95 mask to your appointment. If you do not have a KN95 or N95 mask, KN95 disposable masks will be provided. When arriving for your appointment, you will be asked to wash your hands before entering the office. Hand sanitizer will also be provided along with moisturizing hand lotion. Your temperature will be taken with a contactless thermometer along with oxygenation levels upon arrival.
In addition, I will be disinfecting all surfaces within the office and bathroom between each appointment. To ensure optimal air flow and quality, a Hyper-HEPA Air purifier (IQAir HealthPro Plus) will continuously clean the office air 8x/hr, and two strategically placed fans will ensure continuous air flow. In addition, there is a UV sanitizer installed in the buildings HVAC system. Extra time is allowed between patients to ensure the room is aired out.
I will be wearing a N95 and surgical face mask, eye protection, and apron which will be changed between each appointment. And as always, I will be washing my hands and using hand sanitizer between each appointment.
As always, if you or someone in your household are experiencing cold or flu-like symptoms, even if you have received a negative covid test, we ask that you remain home and reschedule your appointment to a time when you and your household are feeling fully recovered.
